TRANSITIONING OUT OF THE HOSPITAL

Leaving the hospital after having a stroke can be a stressful and overwhelming time. The resources on this page will outline some of the key areas that may need to be addressed prior to/ or soon after discharge. Please also see the resources page for some local resources that may be available in the community.

The Heart and Stroke Foundation of Canada has a comprehensive section on discharge from the hospital. One key resource is the Post Stroke Checklist which is meant to facilitate conversations with your health care providers about common issues post stroke that may persist after discharge.

The Toronto Stroke Network’s My Guide to Stroke Recovery also outlines some considerations for leaving the hospital. The March of Dimes Canada After Stroke Website also has a Hospital to Home page which outlines various resources that are available.
