PRIVACY POLICY and Terms of Use

We collect personal information from health care professionals in the regular course of doing business. This page answers some of your most frequently asked questions, and lets you know exactly how we’re protecting the information you entrust to us.

West GTA Stroke Network respects your privacy. We do not collect personally identifiable information about you unless you voluntarily provide it, such as when you provide email contact information to subscribe to a West GTA Stroke Network education updates or newsletters (via our Mail chimp), send feedback, or subscribe to a course. If you voluntarily provide your email address or other contact information, we might also use it to inform you of changes to the West GTA Stroke Networks work, to survey you about your use or opinion of West GTA Stroke Network projects and priorities. At your request, we will remove your contact information from our files.

We do not make your contact information or any other personally identifiable information available to anyone outside the West GTA Stroke Network team unless we are legally required to do so.

WHAT PERSONAL INFORMATION DO YOU COLLECT ABOUT ME?

We collect the following information about you:

• First Name
• Last Name
• Phone
• E mail address
• Organization you work within
• Health care profession/discipline
• Area of practice
• Region of practice
• Area of Continuum of Care you work in

When you visit our web site, we may also collect:

• information about your computer, including your IP address, the type of operating system and browser you use, and your computer’s location
• what pages you visit on our site and what links you click on (via Google Analytic)

We use the Google Analytics web analytics service, which collects certain data about this website’s traffic via a DoubleClick cookie and the standard Google Analytics implementation.  The specific Google Analytics feature we use is Google Analytics Demographics and Interest Reporting.  Google Analytics Demographics and Interest Reporting enables the West GTA Stroke Network to use information such as user age, gender and interests from Google’s Interest-based advertising or 3rd-party audience data to improve services on the West GTA Stroke Network  website and to help us understand the aggregate demographics of our users.  We cannot and do not obtain personally-identifiable information from Google Analytics.  West GTA Stroke Network does not and will not merge any personally-identifiable information with non-personally identifiable information unless you have been given notice of and have given prior affirmative (i.e. opt-in) consent to that merger.  Google’s ability to use and share information collected by Google Analytics about your visits to West GTA Stroke Networks website  is restricted by the Google Privacy Policy. You can prevent Google Analytics from recognizing you on return visits to this website by disabling (opting out) the Google Analytics cookie.

• We may use other web analysis tools that are built into the West GTA Stroke Network website to measure and collect anonymous session information.
• We also use "cookies" to improve your West GTA Stroke Network web experience and to collect anonymous information about how you use the website. However, cookies are not required for West GTA Stroke Network’s website use. If your browser is configured not to accept cookies, you will still be able to access the West GTA Stroke Networks website and its content.

HOW DO YOU USE THIS INFORMATION?

The main reasons we collect personal information from you are:

• To understand who visits our website and which pages are most useful
• Stakeholder communication
• Registration for events and workshops (information collected via Eventbrite)
• Which course/education materials are accessed most frequently and the  time you spend  in the virtual classroom
• To provide reports to your organizations managers, directors or other leadership regards courses you have completed via the West GTA Stroke Networks Virtual Classroom

When we report information about the West GTA Stroke Networks access, use, and impact, we report aggregate, non-personally identifiable data. Occasionally, we report quoted feedback from users. We do not attribute feedback to specific individuals unless we obtain permission to use that person's name along with the feedback.

USE OF PERSONAL INFORMATION FOR SECONDARY REASONS

If it’s a necessary part of any of these transactions, we may disclose your information to another company (Eventbrite) which is an online registration software used for registering participants for workshops and educational events.

Information related to area of care worked in or organization worked in may be disclosed to the West GTA Stroke Steering Committee and/or CorHealth only for the purpose of demonstrating the number of clinicians educated per year and from which area of the stroke care continuum they service.  Specific names and contact information of participants are not shared with the steering committee or CorHealth.

SHARING OF PERSONAL INFORMATION WITH THIRD PARTIES

We do not share your personal information with any third parties without your consent.

As noted above, some information is provided to West GTA Stroke Steering Committee and/or CorHealth only for the purpose of demonstrating the number of clinicians and from which area of care continuum attended a particular educational event.

West GTA Stroke Network Interpretation of "Non-commercial"

Non-commercial use means that users may not sell, profit from, or commercialize West GTA Stroke Network materials  or works derived from them or works posted on their website created by other partners/stakeholders on behalf of the West GTA Stroke Network. . The guidelines below are intended to help users determine whether or not their use of  materialsfrom the West GTA Stroke Network would be permitted  under the "non-commercial" restriction.

1. Commercialization is prohibited. Users may not directly sell or profit from West GTA Stroke Network materials or from works derived from West GTA Stroke Network  materials.Example: A commercial education or training business may not offer courses based on West GTA Stroke Network materials if students pay a fee for those courses and the business intends to profit as a result.
2. Determination of commercial vs. non-commercial purpose is based on the use, not the user. Materials may be used by individuals, institutions, governments, corporations, or other business whether for-profit or non-profit so long as the use itself is not a commercialization of the materials or a use that is directly intended to generate sales or profit.Example: A corporation may use West GTA Stroke Network Materials  for internal professional development and training purposes.
3. Incidental charges to recover reasonable reproduction costs may be permitted. Recovery of nominal actual costs for copying small amounts (under 100 copies) of West GTA Stroke Network content on paper or CDs is allowed for educational purposes so long as there is no profit motive and so long as the intended use of the copies is in compliance with all license terms. Students must be informed that the materials are freely available on the West GTA Stroke Network Web site and that their purchase of copied materials is optional.Example: An institution in a remote area has limited Internet access and limited network infrastructure on campus, and a professor offers to create CDs or resource booklet of West GTA Stroke Network  materials relevant to his or her course. Infringement Notification

West GTA Stroke Network, prior to making any materials publicly available, has reviewed all material extensively to determine the correct ownership of the material and obtain the appropriate licenses to make the material available on the West GTA Stroke Network website. West GTA Stroke Network will promptly remove any material that is determined to be infringing on the rights of others.
