West GTA Stroke Network respects your privacy. We do not collect personally identifiable information about you unless you voluntarily provide it, such as when you provide email contact information to subscribe to a West GTA Stroke Network education updates or newsletters (via our Mail chimp), send feedback, or subscribe to a course. If you voluntarily provide your email address or other contact information, we might also use it to inform you of changes to the West GTA Stroke Networks work, to survey you about your use or opinion of West GTA Stroke Network projects and priorities. At your request, we will remove your contact information from our files.
We do not make your contact information or any other personally identifiable information available to anyone outside the West GTA Stroke Network team unless we are legally required to do so.

Information related to area of care worked in or organization worked in may be disclosed to the West GTA Stroke Steering Committee and/or CorHealth only for the purpose of demonstrating the number of clinicians educated per year and from which area of the stroke care continuum they service. Specific names and contact information of participants are not shared with the steering committee or CorHealth.
SHARING OF PERSONAL INFORMATION WITH THIRD PARTIES
We do not share your personal information with any third parties without your consent.
As noted above, some information is provided to West GTA Stroke Steering Committee and/or CorHealth only for the purpose of demonstrating the number of clinicians and from which area of care continuum attended a particular educational event.
